Position Summary
Rosalind Franklin University Health Clinic's mobile clinic, Community Care Connection, is seeking to fill a full time registered nurse position. The Community Health Registered Nurse is responsible for facilitating the Community Care Connection mission to provide high quality health screenings, acute care visits, vaccination clinics, and education to underrepresented community groups and patient populations. This role involves direct patient engagement and assistance with healthcare needs and identifying social determinants of health needs. The Community Health Registered Nurse will provide the necessary components of nursing support to patients. In this role you will engage in telephonic and face to face interactions with patients in the community to identify barriers, problem solve and facilitate patient access to healthcare.

Rosalind Franklin University Health Clinics' mobile health vehicle embodies the university's commitment to health equity and access to compassionate, quality care. The Community Care Connection (CCC) travels throughout Lake County to reach underserved populations and offer treatment, prevention and education. The CCC also serves as a powerful educational environment for RFU students who volunteer to assist the licensed providers who staff the mobile unit.
Essential Duties & Responsibilities:
- Facilitating patient registration and scheduling and making sure they are up to date in Athena
- Communicate with families and members of the community care connection team to guide patients to secure their clinical needs
- Collaborate with the clinical lead in oversight of immunizations and preparing vaccines prior to mobile visits including transfers/returns
- Processing patient paperwork post-visit
- Inventory management of office/mobile and clinical supplies
- Troubleshooting operational challenges
- Preserve vaccine ordering and accountability
- Maintain annual calibration of clinical assessment tools
- Assisting provider with all necessary components of Nursing care on the Health Coach i.e. Assessments, vital signs, documentation, patient data and all other duties as assigned
- Initiates, track and reports relevant status updates, data, metrics and other metrics regularly to provide patient safety and maintain HIPAA compliance
- Provide health education workshops to community partners/members - Complete follow-up calls with patients of the mobile clinic
- Connects community members to community resources
- Duties as assigned
Conditions of Employment
- Must achieve satisfactory results from a background check and drug screen
- Must have proof of COVID 19 vaccination and booster or a university approved exemption
Required Education & Experience
- Associates degree or higher
- Current Illinois RN licensure
Required Knowledge, Skills & Abilities
- Interpersonal skills to engage with diverse and underrepresented community groups and patient populations; a strong passion for leadership
- Ability to motivate and coach patients in healthcare literacy
- Knowledge of local and community health resources
- Ability to work independently and be high functioning in a team environment to manage patient and community relationships
- Excellent oral and written communication skills
- Basic computer skills/data entry; MS Office programs (Word, Outlook and Excel)
Preferred Qualifications
- Experience with ICARE and VFC
- Bilingual (Spanish /English)
